Spring Sports Form Information – 2025/2026
Sports physicals for Spring 2026 athletics will be accepted from:
December 16th, 2025 – February 6, 2026
Important:

Parent/guardian signatures on parent forms must be dated on or after December 16, 2025.

Parent Forms signed before this date will not be accepted.
All student -athletes must have a completed and approved physical form on file before tryouts begin in March to be eligible to participate.
Be sure to submit your completed forms to the nurse’s office or place them in the drop box located outside the main entrance of each school.
All necessary forms can be found in the Athletic Handbook. Pay particular attention to pages 17-20. You may also click on the "Athletic Forms and Requirements" link on the side panel (and give the page a moment to jump forward to the appropriate place.)

GENESIS SPORTS REQUIREMENT
In order to be eligible for participation in athletics, the Sports Interest Form must be completed in Genesis.

- Go to Forms
- Go to Athletic Registration
- Scroll down to sports interest and what season
- Scroll down to policies and review each
