- Parent/guardian signatures on parent forms must be dated on or after September 10, 2025.

- Forms signed before this date will not be accepted

All student-athletes must have a completed and approved physical form on file before tryouts begin in December to be eligible to participate.

Be sure to submit your completed forms to the nurse’s office or place them in the drop box located outside the main entrance of each school.
