Sports forms are now being accepted; please note that they must not be dated any earlier than December 18th as per our guidelines. Forms dated earlier December 18th will not be accepted. The deadline to submit forms is Friday, February 9th. If your child participated in a fall or winter sport, they must still submit a sports packet for the spring season.
Please refer to the Athletic Handbook- Grades 6-12 "2023-2024" for more information. Pages 18-20 has physical form information and also instructions for the sports interest form.
Once completed, Crossroads spring sports forms can be placed in the black dropbox outside the middle school entrance from 8:00 am to 6:00 pm, Monday through Friday (with the exception of holidays). Your child can also drop off the forms to their school nurse.
